Mr. Cohen is an elderly man in a nursing home. Stewie Griffin sings "There's a Hole in the Bottom of the Sea" to him and his fellow residents in Whistle While Your Wife Works. After learning it is Mr. Cohen's birthday, Stewie dedicates a portion of the song to him. When an aide wheels out a birthday cake, Mr. Cohen collapses.
